Just a thought, I have to assume that the updates would have been added to a 1.4.4 or 1.4.5 installation during package updates, is that correct?

Yes correct, but there are a few minor features that are not updated automatically.

Running this script in terminal:
$ dash /usr/share/apps/q4os_system/bin/default_desktop_settings.sh
should be sufficient. Note, It will reset all user settings previously made in your profile.

Does this update install the "Desktop Profile Applications" and reset wallpaper, window behavior, etc. ?

No, it only reverts desktop settings in user profile. The tool will be described in the Q4OS documentation soon.

I ran sudo apt-get update and sudo apt-get dist-upgrade, installed updates, but when I run get-q4os-version it still says 1.4.5-n1.
Am I missing something important about the upgrade?

You miss nothing. The 'q4os-desktop' package will be pushed into repositories in a few days.
